What Is PMDD?

PMDD is classified as a depressive disorder in the DSM-5. It involves severe mood, behavioral, and physical symptoms that appear in the luteal phase of the menstrual cycle (the 1–2 weeks before menstruation) and resolve within a few days of the period starting. The cyclical pattern tied to hormonal fluctuations — not to external life events — is PMDD's defining feature.

PMDD vs. PMS

PMS affects up to 75% of menstruating women with mild, manageable physical and emotional symptoms. PMDD affects 3–8% and causes severe symptoms — particularly prominent mood symptoms — that markedly impair work, school, relationships, and daily activities. The difference is not just intensity; PMDD specifically requires significant functional impairment.

Symptoms of PMDD

At least one of the following must be present: marked mood swings; marked irritability or anger often disproportionate to circumstances; marked depressed mood, hopelessness, or self-deprecating thoughts; marked anxiety, tension, or feeling "on edge." Additional symptoms may include: decreased interest in usual activities; difficulty concentrating; lethargy; appetite changes or food cravings; sleep disturbances; feeling overwhelmed or out of control; and physical symptoms (breast tenderness, bloating, joint or muscle pain).

Treatment Options

SSRIs are the most effective treatment for PMDD and can be taken either continuously (daily) or intermittently (only during the luteal phase, beginning 14 days before expected period start). Fluoxetine (Sarafem), sertraline (Zoloft), and paroxetine (Paxil) are all FDA-indicated for PMDD. Research shows both continuous and intermittent dosing are effective — many women see improvement within the first cycle. Oral contraceptives containing drospirenone (Yaz) are also FDA-approved for PMDD. Our PMHNPs discuss all options to find the right approach for you.
